Electronic signature statement
You can sign your housing application on this site. This page explains what that means, word for word, before you sign anything.
How signing works here
When your application is ready, you read back every answer exactly as it will be submitted. You check a box to agree to sign electronically, then type your full legal name. Typing your name is your signature. Each adult in the household signs the same locked copy from their own account. Nobody signs for anyone else.
The words you agree to
This is the signing statement in use now (version 1). You see these exact words again when you sign. Your signature certificate keeps them.
You are signing this application electronically. By continuing, you agree that your electronic signature is the legal equivalent of your handwritten signature, and you consent to receive records about this application electronically. You may request a paper copy of anything you sign, and you may withdraw this consent for future records by contacting the housing authority. Withdrawing consent does not undo a signature you have already given.
I certify that the information in this application is true, accurate, and complete to the best of my knowledge and belief. I understand that the housing authority will verify this information and may request supporting documents, and that knowingly providing false, incomplete, or misleading information may result in denial or termination of assistance and is punishable under federal law, including 18 U.S.C. § 1001. I authorize the housing authority to make the inquiries reasonably necessary to verify this application.
Your signature’s legal effect
Signing this way is legally binding. It counts the same as signing by hand, under the Federal E-SIGN Act (15 U.S.C. § 7001) and the Ohio Uniform Electronic Transactions Act (Ohio Rev. Code § 1306.06).
Your rights
- You can request a paper copy of anything you sign from Demo MHA at no charge.
- You can withdraw your consent to electronic records at any time. Contact Demo MHA to do it. A signature you already gave still counts.
- You don’t have to sign online. Contact Demo MHA’s office to apply on paper.
Your signature record
Every signature is kept just as it was given. Nothing can be changed later in secret. You get a certificate for each one, and you can view or print it at any time.
Each certificate holds a fingerprint of the application as it was signed. Every fingerprint goes into a log that shows any change. If one answer changed later, the fingerprints would not match.
What you need
A web browser like this one, and an email address where Demo MHA can reach you. Your signed copies stay online, and you can print any of them.
